Many nations who can form Italy can be a republic. Venice, Milan, sometimes, Tuscany, the new OPM I guess and also Sienna.
Yet when forming Italy it will always be a monarchy. Previously that what necessary as one of its national ideas was only useful for monarchies which could participate in the HRE Elector game. But as this idea was now changed there isn't really a reason to force the government change to monarchy.
