Mataram missing from Grand Campaign

In my earlier pass I missed a country that should be included in the Grand Campaign selectable list... Mataram (MTR).

Ahh, I used to be in the beta for EU II, but I didn't have time to do anything so I never participated.

I don't want to create several threads, so just combine everything in here

1. MKS is missing as well
2. So does MLC
3. LUA shouldn't be on selectable countries list, since they don't start in 1419.
4. KHM should be removed from revolt.txt - it's an old tag from EU1
