Including landed knights would mean we'd need a title tier below Baron, and each Barony would probably need dozens of "knight slots". It's impossible. Including unlanded knights doesn't really add anything to the game.

As for the great knights, irrc there is an event where a noble distinguishes himself at the court. That should more or less cover it.
