King-Bishop?

So in my Sultanate of Rum game it appears that Scotland came to be ruled by a King-Bishop sometime in the late 1100's. I have a few questions on this and would appreciate any info.

How does this happen?

Is this a monarchy or a theocracy? Or both?

Does the Pope now choose the ruler of Scotland?

Are there any relationship bonuses or penalties for this kind of ruler?

How stable is this kind of government?

It is a theocracy. I think that it arises when someone pushes a bishop's claim to the throne. The new ruler after the old king-bishop dies is a random nobody like when a barony-level bishop dies. Non-religious vassals get a -30 relations hit for the wrong government type. Unfortunately, I have seen them be incredibly stable, and when I found myself a vassal of one, the only faction I could create was "independence," so I couldn't press someone's claim. I had to give the kingdom title to a fellow vassal with the console command.

Interesting. I will be watching this King-Bishopric with interest to see if it take advantage of this stability.

In the game I am currently playing France has had a King-Bishop for over 150 years. I am currently control both the ERE and Hispania empires and I want to take a lot of French territory, but France has not had a civil war in a really long time. They don't seem like they have very many wars at all. I can press a claim here and there for one little county, but then I have to wait years to declare war again or suffer the penalty.
