Or not
The proud republic of Siena had a long history amongst the Italian nations. However, it wasn't until the mid 14th century that they truly rose to power. After quickly crashing their opponents, even exiling the pope himself to France, Siena managed to survive the backlass. Over the years, and with gruelling war, the Siena republic managed to consolidate most of Northern Italy. With the help of France, Spanish Naples also quickly fell.
Despite its wars however, Siena never became an oppressive, paranoid state. Always religiously open (even to perceived heresy, like protestantism, which it eventually embraced) and culturally tolerant, it had a proud republican tradition which almost no one challenged inside the country. The majority of the rulers were just and good and, if they weren't good enough, they would step down without a fight.
Thus came the fateful day of 27th of August 1550, when finally the last core province of the earstwhile Lombard kingdom was conquered. Finally! A unified republic where all Italian could feel at home. Thus the newly elected ruler of the republic finally declared the....feudal monarchy of Italy?
Wait wut?
tl;dr:
Why on earth does the decision to form Italy change you to a monarchy?
