I've seen an inbred from a 1st cousin one time out of 500 hours in the game. So I'd say they're generally safe. Also, far flung dynasty members are pretty safe, I've never seen anyone with just a "Kinswo/man" tag give inbred characters.

Marrying Siblings and Immediate descendants(grandchildren) is not allowed within the game. So that issue is only small.

In my experience...Cousins have a small chance of producing inbred children. Generations of inbreeding will result in increased chances. So as a one off it is not a big deal, but doing it constantly will cause an issue.

So long as you family tree looks like a tree and not a pole you should be fine. I tend to avoid kissing-cousins early in the game to build prestige, but after a while the prestige gain from your own dynasty becomes tempting. However, at that point your family is so branched out it doesn't matter.

I actually did name one pair Jaime and Cersei - they turned out just OK (they shared lustful, skilled commanders, paranoid, cynical, proud and wroth, probably due to their common guardian) - their children came out as inbred imbeciles, though.

The worse I have seen is if you and your child of an opposite have the trait lustful. It may have been a bug, but my ruler had an event were he though his daughter fancied him hmy: Unfortunately I couldn't pick another option, so their inbred child was born.

Even after experimenting with sibling marriages, the chances of their children getting the inbred trait is still pretty low (like, say, 10-20% or something). The chances with cousin-marriages is then also even further ridiculously low. I mean it's low enough that whatever gains you might gain outweigh the risks. If you do end up with an inbred heir they might die young anyways or you can send them off to a bishopric.
