It could potentially fire if you as Poland are democratic and leader of a separate faction than the soviets and haven't been attacked by them and some how the Soviets have some German Territory under their control, if you can manage this then you will get Yalta as Poland.

Go for Polish Revanchism and conquer Lithuania ASAP
Give up Danzig and focus on the Soviets first.
Add AA to your units and only push when you have green air.

Then take out the Germans.
Getting Yalta in this scenario is close to impossible.
