Personally I love playing ironman mode. Every time I want to start a new game in eu4 I play random nation and just give it a go with ironman mode. I play with random new world so I don't play with countries there.

Do you guys play with ironman mode and do you sometimes pick random nations?

I always play ironman. Sure, you can "honor system" play any game one save / never reload without needing a special "mode", but it's nice to know it's actually a design consideration. I think the game plays best that way and feels like that's how it was meant to be played.
