I'm currently playing as an Orthodox Jerusalem. I have Jerusalem (of course), I have Rome, I have Alexandria, and I have Antioch. The first three cities have Patriarchs installed (magically) but for some reason Antioch does not... ....How do I change that? I'm working on restoring the Pentarchy, plan on hitting Constantinople at some point...but..

Well I'm at a loss here, let me know.

You need to auto-generate a vassal (right click) on the temple holding in Antioch. It should be a priest vassal, who will automatically become a patriarch instead of a mere bishop.

I haven't gotten as far as to reconquer one of the Pentarch's...you must auto-create a vassal? Giving it to someone who has a church trait in your court (like my brother) won't elevate that bishopric to a Patriarch?

I was able to install a member of my court (a priest of course) to the Alexandrian throne by making sure I had selected the "Bishophoric of Alexandria" title (which I owned) and granting that landed title to my courtier.

I am wondering if giving the county as a whole to an AI vassal would screw it up somehow. I heard someone say a Miaphysite [sp] Bishop wound up on the throne in his game and it didn't count as a Pentarch due to him not being Orthodox.
