I, as the Germanic ruler of Sweden, married the Christian king of Norway's sister through a favour. More than 10 years into the marriage, she still has given me no children. Is it just tough luck or is there actually a mechanic I'm unaware of that does not let it happen?

Does she hate you? I am pretty sure negative opinion = less chance of baby.
Does she have traits reducing fertility? Or do you have traits reducing fertility? Chaste, Homosexual, Weak, And more that I cant remember off the top of my head.
And are you ever at court? Constantly leading troops reduces chance of fertility aswell, or is maybe she somehow not at your court? Maybe she is your spymaster and in another region?

Traits aside, none of these have any effect on the chance to get children.
Crowded court does not apply to your and your heirs chances(and its from 30 courtiers)
Opinion and location dont have any effect on fertility.

I'm starting to think, despite everyone saying otherwise, that opinion might have something to do. She was kind of pissed of at me because I acknowledged a bastard, so I gave her gifts and honorary titles until she had about +10; I had the "Groom an Heir" ambition and family focus (later switched to seduction), and yet nothing happened. Then I gave it a last shot, switching to carousing and eventually befriending my wife... had three kids in the next five years :)

Yeah, I usually do too, but this time I needed my firstborn to be from my wife because she had a claim on Norway...

I thought so to, but I think it's only a minor effect. Actually, except for celibate and eunuch, I don't think any of the visible things that modify fertility make much of a difference individually (though the cumulative effects can be pretty large--"groom an heir" ambition + seduction focus + lustful trait + spouse with lustful trait add up to something like a +55% increase in fertility. But if the hidden base value is really low or really high, it still might not make a whole lot of difference in the total number of children).

+55% fertility doesn't mean the characters fertility is mulitplied by 1.55.
The percentage is only because the fertility of a character is given as a percentage too.
If a characters base fertility is 50%(the avarage) and all the modifiers add up to +55% than the characters fertility is not 105%. Though I have no idea how exactly this affects the amount of children you have.

So the base ferility is irrelevent to how much fertility the traits add(can be checked using console-->charinfo and then adding a fertility giving trait. Fertility goes up by a flat amount)

Though I have no idea how exactly this affects the amount of children you have.

Each children one character has decrease its fertility, to a point where it can not get any children. Remarrying should get you more child as it's the couple fertility that is checked I believe, but eventually there's a point where I can't seem to get any more kids no matter how many young concubines I get so there must be more to that.

Also pretty sure that opinion affects fertility.
