Anyone know what triggers these missions? I.e. annex a vassal, get a core on a province? E.g. in a current TO/Prussia game I got one to incorporate Pskov not long after I vassalized it, but I've been waiting a long time now to get one for Hamburg but it just doesn't want to trigger.

You have to border the vassal. They also have to be the same religion as you and have fewer provinces. You also can't already have cores on them. I suspect that Hamburg is too far away from you.

But as it's extremely rare that a vassalization offer is better than Unlikely, and failure cost 50 relation, these missions cost a lot of ducats (or Imperial Authority) as well as time and diplomats to complete, while usually give only 1 core.
A better situation is if you get a Conquest CB alongside the mission, because then you don't fail the mission on DoW. Just these CBs are usually for the capitals of non-OPMs. :(

I usually run an elective form of government, so waiting for a shot at a new leader with a decent diplo(7+) rating isn't too bad, that plus high prestige (philosopher advisor, etc) and a diplomat advisor I often get the "likely" icon and usually don't have problems diploannexing vassals if I go that route (which isn't often, as I actually prefer in most cases to keep vassals as vassals. However the -100 penalty to all existing vassals can be painful :( )

These missions are great if you got some OPM/TPM vassals , even better in the HRE , but they cost money ... well you pay money to not suffer the infamy penalty later!

Are we talking about diplomatically offering vassalization to a country or diplo-annexing a current vassal? I thought OP was referring to diplo-annexing a vassal, which AFAIK always succeeds now.

Exactly. Before they added the SoI bonus to diplo skill it was indeed very rare to get better than "maybe" but "high" and "very high" are very common now if you use SoI heavily.

That's really weird, the requirement are very general. It only requires you to border a vassal of your religious group and be at peace. A high diplo ruler makes it more likely to appear, it's base chance is rather low otherwise. I usually get it several times per century, once I got it thrice in a row which was a bit annoying as I had to wait for the 10 year diplovassalizing timer to expire.

There are two 'generic' annex vassal missions.

Both require the target to be an OPM, of your religious group, and yourself is not at war. Either mission hands out free core on the annex.
One of them requires you to not be the Papal State or Japan, and both you and your target can't be in the HRE, but it reduces your infamy by 2 on top of the core you receive.

I suspect you've been receiving missions while you're at war, so it's never popped up

There's also a chance that you're preoccupied by the culture/nation specific missions before 1500, which always takes precedence to the normal missions, then it got drowned in the build manufactory missions after 1500.

The generic_annex_vassal mission does not require the target to be a OPM (and is modified in 5.2 to allow you to get two cores instead of only one). I think you are looking at the annex_minor_mission which has nothing to do with vassals.

Note the manu missions have the same priority as the vassal mission. If you have expanded a lot and don't have many vassals you have a lot more provinces eligible for manu missions than the vassal mission though.
Spending a lot of time at war is probably the most likely thing preventing you from seeing the vassal mission.

The generic_annex_vassal mission does not require the target to be a OPM (and is modified in 5.2 to allow you to get two cores instead of only one). I think you are looking at the annex_minor_mission which has nothing to do with vassals.
