They did not usurp it, they were granted it, which would automatically make them independent-

Not quite correct yourself. k_wales is always given to the heir to e_great britain at birth to help limit the effects of the short reign penalty (since the Welsh nobles would actually have a long reign bonus) and build up stewardship stats. Being king tier this doesn't result in independence and always being held by the heir to e_great britain it always gets inherited when the holder of k_wales becomes the holder of e_great britain. This creates the impression in political map mode that k_wales doesn't exist but if you double click on wales (assuming the holder of e_great britain has a child as an heir, otherwise they hold the title) you'll see that it's held by a king tier caricature and not a duke or the holder of e_great britain.
Maybe you're confused by the title of prince? Wales is a principality and so king gets localized as prince.

Eh, she'd be 101. I've seen characters live longer than that. She's probably abusing the health bonuses you can get from being a hermetic or somesuch.
That would have given them control over Pakistan. Given that it became immediately independent, it seems more likely that they were two multi-king vassals granted independence (possible, since they weren't the same religion). Serene Doge Jawaharlal quickly formed a custom empire and used the customiser to change the name to India.

Well, then, obviously the Nazi Manifest Destiny Invasion of the Soviet Empire was an entirely separate war. Though there must've been some kind of modding done if the Brits and French could keep fighting after the primary defender was defeated.
ETA: On second thought, maybe Grand Prince Adolf just chained Manifest Destiny Invasions and finally ran out of steam because the amphibious assault terrain penalties were prohibitive.
He probably chained invasions - Poland, then France, then UK, with USSR at the same time - and seeing that the ticking warscore and battle warscore was approaching -100% WS, he decided to try and troll everyone by invalidating the war.
