Right, so basically, I WAS playing The King of Ireland. I had control of the whole of Ireland, Wales and Britanny.

My heir, my Grandson was rightfully lined up to take the throne. The King dies, I take on the role of the grandson, only to find that by some strange twist of events, I'm now the Duke of Meath and someone ELSE is the King!

He's not even related to me and he couldn't of usurped the title, as I made sure my character owned most of the counties in Ireland.

I've managed to kill off all of the current kings children, so that I'm now HIS heir, but is there a way to make sure that this kind of thing doesnt happen?

Upon death you play the heir of your Dynasty - not necessarily the heir of your titles. If your grandson was not of your Dynasty you would never play him. What might have happened is that your daughter married someone in a regular marriage (as opposed to matrilineal) so that your grandson became part of his father's Dynasty. You usually want to marry off your elder daughters matrilineally unless you have a few sons.

Pay more attention to the succession for your titles, and the dynasties of your heirs?

I made sure the succession laws were for the eldest son of the ruler to inherit all titles.

Since all my sons had died, my grandson (by my eldest son) then became my heir.

I have played through 3 Kings of Ireland before (in this game) and not had a problem with the succession

There could be some strange bg in 1.7. I had situation when I had multiply kingdoms amd empire title. By some reason, in gavekin empire My 3rd sun was heir and in primogenitive kingdom my first son was heir of main title. I was very confused untill my first son die of natural causes and everything become normal. It was very confusing as I had to make my 3rd some heir in elective kingdoms in roder not to loose my main title. I suspect that game took my thurd son as heir of empire on creation of title because he was heir of primary kingdom title at time of empire creation(elective)

You need alot more detail about the succession laws of the titles in question for us to help you. Also more detail in general. Unless its on elective, the guy who became king had to be related to you in some way unless he usurped the title.

Ok, Now I usurp kingdom of polland and My 4th some become heir of kingdom of polland, when My 3rd sone inherit all other tiitles.

Are you running Gavelkind succession. If you are then your oldest (surviving) son will get your primary title only, with secondary titles going to brothers.
