Please explain your issue is in as much detail as possible.
If you start as a minor monarchy which does not have predetermined historical ruler and ruling family, the rulers consort is married to another noble and has / will have children that are considered his. Even if the consort dies, ruler can't marry again. Tested this with commagene, adiabene and paphlagonia.
Steps to reproduce the issue.
Start as a minor monarchy like commagene, then check the government and click on ruler, it will show the consort as his wife. Then click on the consort and she will have a different husband, if she has children, their father is the noble, not the king.
