I'm guessing that the OP does not controll all holdings in the duchy's.

Check each county, if all Baronies are a part of your realm. You or your vassals need to control ALL holdings in the mentioned duchies, to recreate The Roman Empire. (most of the time the Hashasadins have a barony in The Duchy of Antioch)

Just declare war on them and take the counties and duchies you need.

It's annoying, but you have to look at each individual holding in the county, so you have to click on each county. If you just looked at the map, you'd never have any idea that the pope owned a barony in Benevento, for example. Hashashins usually get a mosque in Antioch by event, so that's a likely culprit. If that's not it, post a screenshot with the tooltip showing what exactly is keeping you from taking the decision.

Actually the holding's holder CoA appears on the map when looking at it close enough, which is really impractical because you have to zoom in really close and it's hard to make them out in a map not cluttered by armies of tiny men and their banners.
