I don't know the exact formula, but here goes: Every member country gets a vote. If they like the emperor they vote yes and if they don't like him they vote no. If there are more yes votes than no votes the reform pass. You can see what the member states voted by opening the log after a reform has been proposed. It is not that easy to read through the log and keep track of the votes at the same time though so pen and paper might come in handy.

Often when I was trying to pass the reform, I pause the game on the following day, if it didn't pas. Then I would go through the log at the bottom of the screen and go through every single vote of the HRE members, noting them all on a piece of paper. Often the votes were something like 23-26, then I would know which members (often OPMs) that I should start to bribe. Often less than 5 is needed. A difference from -200 to 200 in relations can make a big difference in whether they vote yes or no to the reform.

I believe stuff like Royal Marriages with them and vassalization greatly increase chance of being accepted. I also THINK that on the final reform, any state that votes NO, leaves the HRE and attacks you.

The log will show all events that happen in every nation such as other nations going to war with each other who joined in that war who is insutling who and other diplomacy/war events as well as option events like the player will get. The log moves pretty fast if you got higher game speed so best to pause the game just after you started the reform and scroll up to see who voted yes or no.

Also if you don't already know save the game before you pass a reform otherwise its a waste of imperial authority if the reform don't pass.
