Do you mean, how do you access it?
Choose a character as normal -- next to their portrait in the selection screen there's a little round icon. Click it and it'll open the Ruler Designer.
There are three (?) main panels with numerous options on each. Once you make a single change, however, you have actually created a brand new character of his or her own dynasty. You can't use it to tweak historical characters.
