Is it possible that some of them don't spawn at all? I'm curious if leviathan loots are something I can safely expect to acquire by late game.

Yeah that seemed to be the case when I observed some maps using consol command, but this game I've encountered 5 so far. I guess I got really lucky this time.
I believe they get generated after the default empires do, so it may be more a function of how crowded the galaxy is, rather than its absolute size.
I wrote this table long ago in case anyone was interested in the spawn chances.
Note that you will not notice some leviathans spawning at first. You'll never know if the Spactral Wraith will appear until you scan the pulsar that spawns it. You'll never know if the Dimensional Horror will appear until an empire enters the black hole system where it spawns. And you'll never know if the Voidspawn will appear until an empire colonizes the planet that's its egg.
Also Leviathans are coded to never appear in the same cluster as another empire so if you fully populate the galaxy there might not be enough space for all of them.
