In cases like Estonia it's because we want all cultures present in owned provinces to have revolter tags.

How does Paradox decides which countries are added to the game?

In this case yes, but we also add countries for historical reasons. There is no single criteria.

As a fun little anecdote, let me tell you how the Art of War map expansion came about.
As we were wrapping up Res Publica, Johan stepped out from his office, walked over to the EU4 team (which I wasn't a part of at the time but was sitting with) and said, "I want 1000 new provinces outside Europe in 1.8".
I said "That's crazy talk."
He said "Do it anyway."
And then we did (with a lot of help from our betas and map modders).

Ireland is certainly one of the areas that could use map work in the future, along with Africa, Russia and Scandinavia.

In terms of the map, nothing at all. 1.8's new provinces were fantastic and clearly very well researched. We just need more nations in them.
But since we're talking about map changes to Indonesia, how about making the surrounding ocean an inland sea? Naval combat in the region was all about war galleys. Iskandar Muda, Sultan of Aceh and one of the most important figures in the region, relied on his large fleet of war galleys during his conquests of Sumatra and the Malay Peninsular. Besides being historical, it also makes the naval game in the region "better" in that naval dominance isn't determined entirely by who can afford to have a heavy ship. It also gives the natives some ability to fight back against European conquest (though probably not much). Brunei managed to hold their own surprisingly well in a naval conflict against Spain during the period.

Assuming there were in fact any changes to the Russian borders, and you're not all experiencing a mass hallucination (which I'm sure is the case), they're not in 1.15.

I could be playing as Meath. It is technically possible, certainly. Do I look to you like the kind of person who would be playing as Meath? Do you think that is a reasonable thing to expect me to be doing? Is playing Meath something you see as a worthwhile use of my time?
