I've occupied all their trade posts and all they have left is there family hold but the war-score is less then 50%. How do I finish them off?

Vacate the capital so they raise their palace levy with 10ish guys, sail back in and annihilate it. Repeat for incremental warscore gain. Warscore ticks up over time from holding the targeted post too.
