How do I create new countries ?

I want to make Europe as it would have been if WW1 hadn't happened.

So, how do I transfer the control of provinces from one country to another, and how do I rename some of the countries (a big lump of the balkans becoming the Austro-Hungarian Empire, for instance).

Any help would be much appreciated. I know how to open the Revolt file but that's about it.
