Supply limit in Africa is so low I either suffer *insane* attrition or get my small stacks wiped out by massive Carthaginian armies.

I can easily get naval superiority and take over the islands, but the war-score just stagnates at like 26% after doing that, and they don't peace out. It's like 498 AVC. I took out Magna Graecia and Epirus. I also conquered Macedon (so most of Greece), Crete, Pergamon and Rhodes. In this war with Carthage I was hoping to take the rest of Sicily, as well as Sardinia, Corsica, Malta, the Balearic Islands, and maybe even get a foothold in Iberia.

Get a really excellent general so that you can beat the larger Carthaginian stacks with your small stacks. Baiting them into Iberia and blocking them off is also a fun tactic.
 
I have had similar difficulties. Try and get a toe hold somewhere by winning a quick siege, this gives you a safe base where the supply limit is higher. (While sieging the supply limit is x2, I think it is x3 when you win.)
Also worth noting that each port you blockade gives you about 1% warscore so setting up a few right before you offer peace helps.
