This happens with all sorts of wars, but mostly holy wars.
Situation: I'm a member of Religion A. Neighbors of Religion A are all around me, but near both of us, members of Religion B. I holy war a neighbor with 2 or more provinces. I crush their army, start sieging.. And all the counts of Religion A declare and start sieging, and because of the way Holy Wars especially work, they get half or more of the land when I finally peace. This is especially annoying in duchies with even numbers of holdings so I often can't form the duchy.
I get that it's technically realistic for them to pounce on a weakened foe... But why must things occupied by them at peace go to them, unlike any other war type? Why can't the person who did all the work get the land like other war types? It's just plain annoying. At least with claim wars you can kill off the competition, too.
