As discussed in this thread (https://forum.paradoxplaza.com/forum/threads/holy-roman-empire-never-forms.1490267/) AI HRE in 867 (almost?) never forms, or at best, it’s extremely rare.
The requirements are too hard and too specific for AI, especially “+60 Opinion with the pope OR a hook on the pope” part. Other requirement such as 3 kingdoms or level of fame might also need further consideration.
The suggestion is, make the decision more like the CK2 one, where it was deliberately easier for the AI. In CK2 769 start “Form HRE” decision, opinion with pope requirement was only for the players, while 867 version had no requirement about pope opinion.
Pope opinion (or a hook) requirement makes sense from a historical/role-play perspective, but atm it makes HRE never form by the hands of the AI. Until some better methods are implemented (HRE flavour pack or something), I would make the decision a little bit easier for the AI.
Of course, HRE shouldn’t form every game, but from time to time it really should. In my humble opinion, it worked better in CK2.
