"Create Pan-North American State" decision does not appear [Windows][1.10.7][02cc]

Description
Going monarchist UK, did all focuses including unite the anglosphere, DOW Canada and made them a puppet, DOW USA and annexed fully. Decision did not appear even when rolled back to 1.10.5.

Did NOT take decision to foment rebellion, did not give them Labrador and Newfoundland.

However, when turning of ironman and activating focus.nochecks and focus.autocomplete, the decision appears without fail.

Steps to Reproduce
1. Do all usual steps to put Edward VIII on throne
2. Complete focuses to get wargoals on former dominions
3. DOW on, puppet Canada in peace deal
4. Complete Unite the Anglosphere, declare war on and annex USA fully.
5. Decision does not appear.

This still occurs in 1.12.6. Can confirm no Pan-North American decision appears even after putting Edward on the throne, puppeting Canada, completing all focuses and annexing the US.
