Im tempted to buy the new DLC , but there are a few things I want know before...
How are the diseases spreading?Its random?Its there anything you can do to stop the disease from spreading to your realm or to your dirrection?

When you start a new game you can choose disease settings in game rules, if im not mistaken game rules was added in the patch and doesn't require dlc, here on the wiki you can see the difference between the settings http://www.ckiiwiki.com/Game_rules#Reaper.27s_Due:_Major_Epidemics.
The diseases starts in a county and then spreads until it dies out. How often this happens depends on your settings. You can build a hospital in counties to fight against the diseases, the hospital is a new slot like forts and trade posts.
This is a great dlc btw, absolutely worth getting.

Its true that the death rate its so hight that It makes the game unplayable?

Unplayable, absolutely not. It might be a little high or i might just be unlucky. Heres my last 4 characters death ages and reasons.
Died of cancer at age 51
Died of Was murdered at 31
Died of Great Pox at 35
Died of Severe stress 52
Makes the game more difficult though which is nice.

Edit:
If im not mistaken the average age of rulers historically was around 50.

The first years after gamestart are hard and sometimes are a real gamble. But after that it is okay and your dynasty will survive minor epidemics. Black death can wipe out whole dynasties if you are not carefull and close the gates. But it is also a way of killing unwanted vassals and famaly members (placing them as a commander or just not closing the gates ect.) The only thing that is odd is, that posessed and mad seems to be over the top. Nearly one out of 4 characters seems to be crazy. But that gonna be fixed soon i think.

I'm pretty sure that a disease spreads pretty logically along counties. By increasing the disease resistance in a county it becomes a lot less likely that a disease spreads to your county from a neighbouring county. The hospital upgrades needed for that are expensive but they definately work as intended from my experience.

That's why when the Black Plague hit my current campaign I intentionally avoided do anything that would raise my realm's prosperity. I only had a basic hospital in each county of my duchy (Lombardy) and yet somehow the entire plague passed me by.

Not the best plan. Prosperity makes it cheaper to build buildings making it easier get higher tier hospitals put up. It is only a 20% penalty, compare it to hospital level (level 1 provides no disease resistance)

level 3- 15% (20%)
level 4- 30%
level 5- 50%
level 6- 75%

It was a bit late for that - I was talking about random events firing as the plague wrapped around my borders. The option came up twice to increase prosperity and I politely declined both times, at least until the plague passed me by. After that of course I was more than happy to allow traders and merchants to go back to wandering the realm, but before that it was more important to stop the plague hitting than to defend against it coming.
