When I approach Cairn, by Caidus and Radix, I hear the sound of a hidden item being revealed. I spent nearly 10 minutes checking every freakin pixel here and I can't find where it is. Can someone help me?

Same here. I think it might just be an indicator that you can loot one of his parts - each is a unique Accessory with a stat benefit that can either be improved at the forge or made into a special Artifact. For me, I took the head, which yields a staff blueprint.
I assume you get a part after completing the quest, and not the first time you get there?
I sided with Chorus, so for me they were the same. Crimson Spear (forget name) suggests I take a trophy after disrupting the ritual, and then you get your pick of his body. No idea how it plays out for Disfavored, since from what I've read they all but require you to spare Cairn. I know you can kill him on Anarchist also. Haven't a clue about Rebels.
It's one of the small purple stones on the ground (which is why you can't see the people highlight on it).

Could have sworn I took a screenshot of it. I still remember where it was though, so if you have one of your own I can circle it for you. There was only an emerald in there, at any rate.
I found it, it's inside the purple lump of crystal behind Radix and slightly to the right. Not sure how I missed it. I should probably go to bed.

I had the exact same problem. Took me waaaaay to long to see it. Glad I am not the only one.
Yeah, was the same for me, I could only find it after the big event, when the whole region had this greyish taint and so the purple highlighting was again visible
Had the same problem. Eventually found it, once the people moved out of the way.
But... Also heard the sound inside the spire room, couldn't find anything there. Can anyone point that one out for me?

Actually, if you run with Disfavored, Radix wants to kill Cairn as well to blight the land and starve the Chorus. It seems that either way, Cairn dies.
