Just noticing since the last patch or so that diseases don't seem to come through much anymore...

edit: I think I found out why. The threshold for overcrowding in courts is significantly larger- seems like it was a disease trigger? Seems interesting- I might lower that threshold. 75 people in a court? Not very common for my court.

CROWDED_THRESHOLD_MODIFIER was new in 2.4x and got raised form 50 to 75 in 2.5x.
Before 2.4 i think disease was instead based on COURT_SIZE_CHILD_PENALTY_THRESHOLD (and income of course, as now).
So i think...

Wow, the threshold was 50? I thought it was 30, or is there a separate threshold level for fertility? The one I'm referring to pertains to fertility and how apparently it's harder to have children if your court size is 30+.

.. is there a separate threshold level for fertility? .

COURT_SIZE_CHILD_PENALTY_THRESHOLD which hasn't changed as it's still 30.

The OP refers to this in defines.lua
Code:
NDisease = {
    SMALL_TOWN_INCOME = 11.0, -- A coastal town with this income has no chance of starting an outbreak
    BIG_TOWN_INCOME = 30.0, -- A coastal town with this income is always a candidate for starting an outbreak
    MIN_OUTBREAK_CHANCE = 0.1, -- Min chance that an outbreak will happen in a particular town
    CROWDED_THRESHOLD_MODIFIER = 75, -- How many courtiers in a court to make it crowded and increase chance for disease.

while fertility has several lines affecting it, among those are these
Code:
BASE_NR_OF_CHILDREN_PER_COUPLE = 2,
EXTRA_NR_OF_CHILDREN_FOR_PLAYERS = 2,
COURT_SIZE_CHILD_PENALTY_THRESHOLD = 30,

I got hit by Camp Fever. Killed my Emperor and his son inherited. The son died in 6 months, and his son took over.

THAT KID DIED IN 3 MONTHS.

Now I'm playing a 3 year old empress. :/

Well I passed by a sign on the way to work advertising youth soccer leagues for ages 2 and up, so if a 2 year old can kick a soccer ball, I know your 3 year old empress can point fingers and speak, so have at it!
