There is a game rule setting to reduce epidemics. Which isn't really needed if you play somewhere inland. But it's very nice if you play in coastal areas like Italy or Greece. You still get epidemics now and then, but it's not a constant issue and they are manageable with hospitals and seclusion.
As for diseases, the only really annoying one is great pox, which happens far too often. You'll often see young people get it or historical characters. Should be toned down. But the others offer some nice flavor. Even cancer doesn't have to be a death sentence. There are cures of a sort and if you're lucky and stack other health bonuses you can live with it for many years
