It's hard to tell without better resolution, but my bet is that the neck things move like mandibles, and the mouth is between them, or possibly at the end of the chin thing.

I think this question is kind of rude, but his mouth might be under or is his little trunk. The neck seems....sub-optimal for a mouth, especially since there look like little appendages that might bring food to a pocket under the short trunk.
