Suppose an entire royal dynasty dies out. What happens to the title? Is someone created randomly, does an election take place among vassals, or...?

Most dynasties have intermarried with other dynasties at some point, so in all likelihood the title would pass to another dynasty before the house even became extinct.

I think you mean what happens when all people in the line of succession have died. If the last title holder is not independent, his liege inherits the title. If he is independent, one of his vassals gets the title. If he has no vassals, one of his courtiers gets the title. If he has no courtiers, one is generated.
