I'm a big horror fan, and though I know it might not be universally popular with the CK2 player base, I would love to see a Horror/Halloween-themed DLC. (And to answer critics, a Halloween DLC would have an historical basis, though one rooted more in European folklore than pure history).
Possible content:
1) Zombie Outbreak/Plague/Apocalypse Event (spreads like an epidemic).
2) Ruler is a Vampire (quasi-immortal, negative opinion modifier due to feeding on subjects, factions form to kill him with stake through the heart).
3) Lycanthropy (negative trait, ruler turns into a wolf when the moon is full).
4) Baron von Frankenstein (mad scientist moves into your county, terrorizes peasants with grave robbing, monster escapes causing rebellions).
5) Elisabeth Bathory (aristocratic serial killer depopulates the countryside).
6) Mummies (in the near-east, disinterred Pharaohs go on murder sprees).
7) Witch/Warlock (trait, Intrigue bonus)
8) Sell Soul to Devil aka Faust (event, large positive multiplier, but with a future cost)
9) Golem (event, runs amok devastating cities)
