Not long time ago there was a decent opinion modifier called 'Competing Great Power' between those countries who managed to reach the largest score.
It was used to make allying between strong countries harder, but was removed in last patches.
I'd like to suggest mechanics based on the same principles of evaluating countries by top score they achieved, as I personally find scoring system in EU4 underestimated and super potential.
So, introduce you Great Power mechanics.
I. Becoming Great Power
Country gains Great Power status, if all of the next conditions are true:
1. There are less than 10 great powers in the world
2. Country score is greater than 50 (to put great power status obtain aside from the game start)
3. Country score is in top 10
Country loses Great Power status, if it leaves top 15.
The idea is to make Great Power list change smoothly. If country A enters Great Power club, it must lose position to 5 another countries before it loses Great Power status.
II. Special Rules
1. All Great Powers have opinion malus with each other.
2. Alliances between Great Powers are hardly limited, only 1 alliance is allowed with another Great Power.
3. Great Power can be only rivalled by another Great Power.
4. Great Power gains special Conflict of Interests (CoI) Call To Arms when another Great Power attacks any country with shared border.
Accepting CoI CTA will make defending Great Power co-belligerent (like Emperor defending HRE) and give considerable bonus (+prestige/Power Projection/MP recovery?...)
III. Great Power Bonuses
I think any moderate military, diplomatic or economic bonus will make sense.
Personally, I find the next list acceptable:
1. gain Empire rank automatically if possible
2. +10% Morale of Armies
3. +1 Diplomatic Reputation
4. +1 Yearly Prestige
IV. Impact
Great Power mechanic can protect smaller countries from larger countries ambitions, effectively managing power balance in region.
1. Weak countries feel more protective from strong ones.
2. Strong countries (which player's country is in most cases) feel more challenge, facing resistance from another strong countries during expansion.
3. Get rid of absurd France - Austria - Commonwealth alliances both between AI coutries in SP and in multiplayer.
V. Possible Improvements
1. Coalitions/Aggressive Expansion mechanic can be completely replaced by Great Power one, which doesn't use hardcoded number (50 AE) but tries to make resistance to aggressive expander with much more flexible diplomatic/alliances system.
Small countries won't face any problems (except standart alliances, lack of MP, money etc of course) expanding here and there, till they become Great Powers.
2. '1 Great Power Ally' hardcoded limit can be tweaked and influenced by Diplomatic Reputation value or any other way which makes sense.
3. Conflict of Interest 'shared border' rule can be improved somehow. The main idea is to give one Great Power some way to prevent another Great Power from expansion.
4. Great Powers total number should be tweaked.
