Aye, my suspicion is that it is something related to (re)-rendering 3D objects (or their animations?) while changing between system and galaxy view, but again - I have never been able to reliably reproduce the issue. :(
I can consistently reproduce as well by blowing up a planet with a planet cracker and then zooming out right before or after it actually explodes.

Had this happen during a MP game, I adamantly avoided entering the planet's system so it wouldn't disappear. Though I don't think I terraformed it or anything?

If I am terraforming a world and when the terraforming completes, if I leave the system view before the animation to change the planet into its new type is finished then the planet will end up on the galaxy map. Going back into the system view of that particular planet removes it from the galaxy view.

I didn't even know there was an animation for terraforming. :oops: Mystery planets solved, I guess, now I know how to get rid of them.

You can reliably recreate the bug by changing planet type in console "planet_class pc_tropical," then immediately zoom out to galaxy view. Only way to remove that I know of is saving then loading the save.

Had it only once for planets, but switching to galaxy view upon ending a battle would have wrecks and monster corpses floating giant on the map. Long time I didn't see this bug though.

I've got this bug back in early 1.x versions with dying ship hulls after big battles.
Without any doubt the bug is connected to perfomance drops.
You will have to use some 2400+ savegame on largest galaxy on medium pc with a lot of large fleets moving here and there to reproduce it I suppose. Follow the white rabbit fps drops.
