Ck2plus at least does the second (it costs piety or prestige to beg your head of religion for the right to do it)
Prestige for a justified holy war, in which no piety is expended getting it. However, it really doesn't matter, because the sheer piety you get out of beating religious enemies in CK2+ is ridiculous. You can easily make 400 piety in a decent war, and much more from a large battle (over 10k enemies and a win).
