Put my fleet in a very friendly port (my personal demesne), and the game claimed that they were not in a friendly port, so some would be lost.

A count in the duchy of Damietta was warring against the Duchess as part of a plot to take the duchy. I can only guess the bug has something to do with that. That or the thousands of foreign troops that like to hang out on the duchy of Sinai.

You get this message when a province where you recruited part of your fleet is under siege. It is a bit misleading. It should mention the home port as the reason for this.

I've run into this problem as well... I even divided the fleet up to individual units and sent them to the County of origin, without success. I'm in no conflicts (Crusade just ended) and there are no sieges or enemies in my realm.

Is losing ships like losing men? Can they be re-raised later, or are the galleys gone gone?
