For some reason my game always defaults back to 50Hz in the settings. I don't know why it's doing this. I have always had a 60Hz monitor. I have an Asus PB238Q.

Please attach your Documents/CK2/settings.txt. From your Documents/Paradox/CK2/logs/ directory, post system.log .

As you can see, the game detects that 1920x1080 is supported at 50, 59 and 60 hz. And it initially picked 60hz there, and that settings.txt shows it using 60hz.

Where exactly are you seeing it at 50hz?

If i recall correctly that's could be an issue in some specific mode, windowed i think..
It's only the displayed Hz number that is wrong, while it actually is at 60.

At least that was an officlal explanation once.
A long time ago in another universe...

Could be related.
Just a guess on my part.

I upgraded to Windows 10 and installed a new Samsung 850 Evo SSD. I'm still having this issue though. It still says 50Hz.

They certainly can, but I don't believe they'l have any effect on the refresh rate.
