I just recently bought this game but would love an expansion :)
More help from the interface for character management (a flag notifier at the top of the screen for when you have people wanting titles or when they've just turned 18 for instance) would for instance be nice, more missions for all nations, etc.
As many said in the thread where paradox checked the interest for another eu3 expansion a new expansion for rome would have to add something more. The more expansions there are the more stuff would need to be added (and with stuff I mean concept additions and changes, not just text file additions/changes, even though they are also nice).
That thread however also contained a number of such suggestions (and the feature list for HTTT actually reads very much like what was suggested in that thread) so I'd guess the probability of a eu:rome expansion would go up if there where some nice clear ideas about what they could add. So keep adding more of those and we could perhaps hope at least ;)
Personally I don't see an easy way to extend the timeline (possibly it could be made to start earlier), the map however could easily be expanded atleast a bit eastward.
First though I'd very much like the crash on exit bug fixed, as it makes other things unstable untill you reboot and imho is a rather serious bug (not one I'd expect to pay to get fixed).

If the map was extended eastwards the game could be made to start a lot earlier and include Alexander (or atleast as other's have mentioned the diadochi period, which would fit the game well possibly with a few more additions)...
Other than this a nice interface change would be to have a tab for sorting according to age as well as ambition, etc in the character list tool. This would make it a lot easier to see when people are about to come of age, be able to be made magistrates, etc.
Also some more hot keys within the interface such as "A = sort by ambition" would be very nice, this game is very click-heavy...

EDIT:
And it'd be nice to have a proper roman senate for the roman player, i.e. 2 consuls, the ability to proclaim someone dictator legally for a limited amount of time in desperate times, etc.
