Well, no kidding. There is no difference in vanilla, besides the de jure land and cosmetics.
Vast difference in CK2+ and HIP, though. You will be crowned by the pope and get the decision to partition the kingdom of Germany for one, and HRE emperor has access to the decision to restore Roman empire
