As simple as that but USA ruler will be very strange because rulers starts in 1700 in monarchs.usa and not before. Nevertheless, first ruler is easy to extend (same date principle) or you can create new ones.

I'd most likely just have the ruler be some form of Council which lasts an extremely long time. I'd probably make their stats 4 stars for diplomacy and economy and 3 for military.

Does that mean that USA can't form in for instance, the Inca Empire is still alive and kicking in the 1770's?

It means that the USA can't become independent from the Inca Empire. Really only an issue if a human is playing them. :p
