This has been a recurent question in the General Forum but I thought this would rather belong here. So here it goes:
1. Straits are defined in two different files: \Map\adj-defs.csv contains the strait's *code*, e.g.:
and \DB\straits.csv contains the strait's *graphics*, e.g.:
where each couple of numbers stands for the coordinates of one little rock in the sea.
2. To remove a strait, just delete the corresponding line in adj-defs.csv which is easy because there are comments, and the corresponding line(s) in straits.csv which is tricky because there aren't any comments.
To help people out, here is a commented file that I found on the forum soon after the beta patch making straits moddable was released:

To add a strait, you have to add a line in adj-defs.csv which is easy because you can find provinces and sea provinces IDs in province.csv, and as many lines as you want rocks to appear on your map in straits.csv, which is not so easy because you have no idea as to how the coordinates fit on the map.
Here are some I added myself. It is rather a tedious work to find the right numbers for the right place so this may save you some time if you thought about adding them yourself:
