To be honest I can't answer about it still happening as I've not played the game in a while.

From memory the bug randomly occurred. I couldn't pin it to anything specific. I managed to fudge my way through it by using the shotgun blasts which targeted an area killing the enemy I couldn't directly target. It didn't happen with all enemies on a level but usually was limited to one enemy character. This issue occurred atleast 4/5 times. I completed the game one time and stopped playing after my 2nd play through was corrupted preventing me playing on.
