We all knew about kingdom of Jerusalem, counties of Edessa, Tripoli and principality of Antioch. If crusades was more successful all this crusade realms should merged into one state - Outremer. For creation of this empire you should make decision "Unite Crusade states" - hold de jure kingdoms of Jerusalem, Syria and Egypt and have 2000 prestige. After creation all rulers of Outremer gain trait "Crusader Emperor" - which provides +10 relations to vassals, +5 monthly prestige, +10 monthly piety, +15 relations to Pope.

The region was called like this in the Middle Ages. Even German texts in the Middle Ages called it 'Oltremer' or some varities of the word.
