Thought this might be an interesting idea if possible. If a disease becomes an epidemic or a plague it could get a unique name rather than just smallpox, measles etc.

The epidemic could be named after a common symptom of a disease, the country in which it first takes hold or the first titled victim of the disease.

The disease name could then be found in the epidemic map menu or the chronicle.

This would've worked well with the death counter they had but apparently that slowed the game down and they had to remove it.

except Justinian didn't die of the Plague, as the OP's suggestion would demand in order to be named after him, though he did catch it... so maybe the OP's suggestion that epidemics be named after a titled victim should be adjusted to be something like "highest ranking top league who's territory suffered" or "ruler that controlled the most infected provinces of the outbreak"

Another option, certainly.

The example I referred to was based on the Buide Conaill "Conall's yellow" a devastating 7th plague in Ireland supposedly named after its first victim.

Colours as symptoms, red for rash, yellow for jaundice, black for gangrene could also be reflected in the naming system.
