It really doesn't, but because provinces have to be grouped in states you get some questionable stuff with islands. Maldives should either be merged with some state in southern india or maybe they could make some one or two province states as an exception. Indian nations can "abuse" the Maldives by putting it into a trade company and getting the +10% trade value investment, affecting the whole Coromandel (which is pretty rich). You get the same with ceuta, anatolia, and some other niche examples.
They shouldn't be in india because then true heir of timur would require their conquest

There are two province states in the game already, for instance the Falklands+South Georgia in the South Atlantic, or Greenland. So I don't see any reason why this couldn't be implemented.
