I actually ONLY play starting out as count. It gives that unbeatable "start small and build an empire from scratch" feeling.
In my last campaign I made the counts of Savoie into kings of Spain, North Africa, Naples and Syria, for example. I was slowly moving in on the crown of France too when I froze the campaign due to not having time to play. Lots of fun!
Starting at count is tough, but I'd say it's definitely the most fun way to play.
