So I've been thinking about some ideas for Doctrines that I want to put out there.
The first idea is adding a couple of extra levels to the Crime Doctrines: Dispensation and Religious Use.
Dispensation lies between Criminal and Shunned. Basically the act is normally considered Criminal but it is possible to secure a special dispensation from either you Head of Faith or your/your liege's Realm Priest in exchange for Piety, and maybe some gold and favours, which makes it only Shunned. This dispensation can be removed, so it pays to keep in the good graces of the religious authorities.
Religious use basically means that the activity is permitted, or even required, in some limited rites and festivals but is otherwise shunned or forbidden. An example might be a particularly puritanical sect of Christianity that bans alcohol except for mass. It might not work for all possible Doctrines though.
The second idea is for a Realm Law dedicated to enforcement of Public Morality according to your Crime Doctrines. This has three levels: Relaxed, Normal and Strict. At the Normal Setting the realm will simply regard acts as Criminal, Shunned or Accepted according to their Doctrines. If their approach is Relaxed then anything that was Criminal merely becomes Shunned and Shunned acts become Acceptable. A Strict approach means that anything that was Shunned becomes Criminal, anything Criminal becomes subject to harsher punishments and Dispensations might be ignored. Raising the level grants Piety whilst lowering it costs Piety. Zealous lose stress for raising it and will approve of rulers of the same Faith/Religion who are Stricter. Feudal Vassals are compelled to follow their Liege's Enforcement Law, though they can gain a Right that grants them autonomy in that regard, whilst Clan and Tribal Vassals will follow their Liege's rulings if they are above a certain level of opinion but are otherwise free to choose as they wish. Public Morality level could either be across the board or it could be broken down to particular acts. There could also be a mechanic that causes the enforcement level to shift towards Relaxed over time if the ruler has low Stewardship.
Finally, I have some ideas for some new Doctrines, mostly to do with crimes.
Firstly, the only non-crime one, there should be a Doctrine on Pilgrimages, at least for Abrahamic religions, as there were a few Islamic and Christian sects that disdained them. These are currently hard-coded in the game but having an option to toggle it on and off could add for some more customisation.
Secondly, there should be a Doctrine for the Faith's stance on Usury. In addition to the normal levels in the game and suggested above there should also be an Infidels Only option which allows you to lend money to people outside of your Religion or allows people from other Religions to lend money in your Realm if they are allowed to by their Doctrines. Allowing Usury can open a number of methods for making money or boosting through lending to other characters or in vesting in merchant ventures, though these can backfire and may make you very unpopular, even if it is an acceptable practice.

- Drugs (also makes Hashishiyah, Henbane Addict and Opium Addled a Sin or Great Sin depending on the level)

- Self-mutilation (also makes Flagellant a Sin or Great sin depending on the level).
