Ohhh yeah !! that would be so cooooool !!
Cool aside, it should be suitably catastrophic.
There is a masterpiece of Song dynasty art (i.e it is concurrent with CK3's timeframe, China art pack please devs) called the 清明河上图,which depicts the capital of Northern Song, Kaifeng, at its heyday.
It was painted because the Yellow River destroyed most of the city in one of its hundreds of floods, and the artist was doing his part to preserve the memory of the city, IIRC.
Heavy stuff aside, ten years ago at the Shanghai Expo, IIRC, the painting was rendered into a huge dynamic digital art on display in the China pavilion (which took inspiration from the Tomb of the King of Ma 马王堆), where you could see all those painted figures going about their daily business, buying tea and food, walking around etc. I think you could find recordings on YouTube.

Music should be part of an immersion pack or bundle. It is annoying to buy so much DLC separately.
In regard to how specific some are best have them alone honestly.

- Start as an adventurer. I would just really like to see this option, not sure it's worth 30 bucks. Maybe combined with a iron man proof Ruler Designer and some other mechanics. Maybe even design a culture and religion to start with, from scratch. So you start out with a caravan of fictional people with fictional culture and fictional religion.

- A dlc that has a big focus on art. Really building a cathedral over multiple generations, starting universities, a sort of race to the Renaissance. Also add science to this, printing press, gun powder, etc, church reformers, free thinkers, humanists, etc.

- Natural disasters could be interesting. Like earth quakes, volcano outbursts and big storms and floods changing coastal territories.

- Adding North and Central America could be fun.

I'm guessing CK3 DLC's will come with start dates attached.

ERE+imperial government rework comes with 1081 start date for my boi Alexios.
Nomadic government+reworking of contracts for nomads and tributaries+trade rework comes with 1220 for Genghis Khan
936 start date might come with a Europe focused DLC in the end, after DLCs have buffed every other region and Europe needs an overhaul.

Nah, I guess something snapped in me when you said natural disasters are cool, so I went off course a bit.

(And Chinese art pack, since the map is so much to the east already, some stand-in events for China should be in place.)

I 'd suggest 680/10/11, the day after Hussayn's death. I 'd say the second Fitna seems to be a better chance to rebuild Persia. Shi'as, Khawarijs, as well as bedouins in Mecca are all willing to have their own caliph rule the Arabian empire. In Africa, berbers are also ready to push muslims back. While in Byzantine Empire, Asparukh have just started his invasion. Pepin the middle also come into power in this very year.

Late antiquity history especially middle eastern history from that period is like a black hole !! nothing survived from that period no scrolls no books no nothing , and all that "early Islamic/arabic" history is nothing but rumers and made up stories from later abbasid years.

For more on this topic I highly advise you to read (In the Shadow of the Sword) a book by Tom Holland , which covers what we have on that perioud of Byzantine/Persian history and clarify the darkness of early Islamic/Arabic history.
or watch his documentary about this very subject here

All in all it's a very interesting time perioud cuz it's shrouded by religious myths and mystery which is a fertile ground for any historical research.
And adding it to our beloved game will be a huge challenge or nearly impossible from a historical pov.

I'm assuming big expansions.

DLC: Expanded content for city states, including political game with neighbouring powers and fleets.
Update: Expanded contracts that allow a larger spectrum between proper feudalism and similar systems, allowing each state to change dynamically from or toward feudalism.

DLC: Expanded content for the steppes, including nomadic alliances mechanics, where everything from religion to marriages and military power works a bit differently. Nomads also have a different "presence" on the map.
Update: New ethinicites all over the map, and new religion mechanics that allow minority religions and cultures in each county (works great to represent the diversity within nomadic hordes, but opens new opportunities in other parts of the world as well).

DLC: Libraries and artifacts. But unlike CK2, this time it's only about the really rare stuff (so no "common swords", every ruler is able to afford that stuff...). Additionally and over the course of the game, each dynasty will build a dynastic library. It would work like the Legacy, but instead you'd build a library and fill it with books. Books can work in different ways: stat boosts, but also by replacing a specific trait when it's needed in an event. They can also open new opportunities by spending lifestyle points in reading them instead of spending them in unlocking lifestyles, like unlocking new traits or providing powerful boons. Artifacts work in a similar way - they don't just provide basic stats, they also alter the way you play with them. But people expect certain things from someone wearing a proud and fancy crown, so expect to get some stress if you don't behave like people expect your character to do. Artifacts are tied to characters and not dynasties.
Update: Some lifestyles stuff?

DLC (or stand-alone): Tales of the Summer Court - a bit like the world randomizer of CK2, but with fantasy content. The map will be filled with appropriately medieval-themed fantasy races - so no orcs and goblins, but Irish Sidhe and Fomorians, german kobolds, djinns, nordic elves and giants etc. The base game mechanics are exactly the same, but with a different skin.

I think you make an excellent point regarding artifacts.

1) Make them exclusively rare. There is a ton of clutter in CK2 that makes very little sense. At the absolute worst you are the lord of powerful castle, and overwhelmingly, people will be lord of more than just a county capital. You're going to have access to some books and a nice sword. That shouldn't be something the game needs to remark upon.

2) Give some relics a mechanical role. Some should be more than just stat boosters.

I view as must haves. I can virtually guarantee they will be present.

1. Byzantine Empire focused DLC.
2. Great Wonders
3. Artifact/Items
4. Societies
5. Legendary Bloodlines (This would probably work within the confines of the existing dynasty tech system, but as opposed to general dynastic growth, you receive bonuses for specific deeds your ancestors have accomplished. That may not be terribly realistic, the fact that my dad was Alexander the Great doesn't actually mean I'll be worth a damn as a fighter, but as a game mechanic that breathed life into all portions of the game, but most critically, the late game.)

I can just about guarantee all of these will be present, though if I had to pick one to not make the cut it would be five. The other four I would be blown away not see, though I think the odds are good we get some version of five as well.

Except they had nothing to do with the medieval old world. Indonesia, Swahili states, Zimbabwe? Sure, they were part of the Indian Ocean trade route and the already playable Indian rulers could be interested in maintaining their interests there. Japan, sure, because it was feudal and very CK-esque. But I don't see how Americas could be relevant for the gameplay. I'm willing to accept Greenland due to Viking settlement and potential cool interaction with Innu, but anything more west could be handled via offmap events such as "Skrælings stole our drakkar and now their adventurer came to loot us".
