I think the title is self-sufficient, but to get the Deus Vult achievement, do I need to actually colonize the 4 Holy Worlds, or can I consecrate those 4 worlds with the AP?
Deus Vult - Do you have to colonize? Does consecrating count?

You need to start the colonization process on all four planets for the achievement to unlock.
Once you consecrate a planet, you can't go back on it, right? Or can you remove consecration status?
You can remove consecration status for some influence, or perhaps unity now after the last patch? One of the two.

As VidarUlv stated, you need to start the colonisation process.
If you are not above cheesing achievements, you can start a sandbox ironman game with no ais but the Fallen Empires and then simultaneously colonise all holy worlds, that is how I did it.
It is unity now.

Thank you both. I'll just play a normal game and get those planets in the end. Thank you.
Word of advice: I've had some trouble with the AI renaming these planets, making them impossible to locate via the Find menu. I had to inspect the save file to uncover their new names later.
