Poland-Lithuania, Novgorod and a northern german state like Brandenburg is enough to withstand a rebbelion from Sweden. After the first war just placate the leaders and just declare war a lot to keep their manpower low. By the start of 1500's, all of Scandinavia will be yours! And you will be a great power! Well, with 30 troops at your disposal...From what I have seen, the problems seems to be that Denmark can't get any decent alliances, whereas Sweden always gains either Poland or Lithuania(or both) and sometimes Muscovy and France or Austria.

So from the very beginning of the term, Denmark has been part of it. And that beginning lies after 1658.
If you want to go before that, "Scandinavia" doesn't refer to the peninsula, but is a vague term roughly similar to Scania. Either Denmark is part of Scandinavia, or Stockholm isn't.
